FEE POLICY
ACTSolutions

To insure your understanding of ACTSolutions office policies, I ask that you read the following information.  Please sign your name to indicate your understanding and agreement to abide by such policies.  Should you have any questions, please feel free to ask.

1. All paper work is to be read and completed prior to initial intake.
2. Full payment for each sessions is required at the time the service is rendered.  Payment may be made by cash, check, credit card (Visa, MasterCard and American Express).
3. At this time, ACTSolutions will not file with your insurance carrier for you. You will be provided with adequate information on your receipt that if you choose to file, your insurance company may reimburse you.
4. Your appointment time is reserved for you.  If you are unable to keep your appointment, please notify this office at least 24 hours in advance.  See LATE CANCELLATION/ “NO-SHOW” POLICY.  In the event of no notification, you will be responsible for the missed session.

Individual Therapy Session (45/50 min)                     120.00
Family/Marital Therapy       (45/50 min)                     120.00

Individual Therapy Session (30 min)                            75.00
Interactive/Play Therapy      (30 min)                          75.00
Interactive/Play Therapy      (45/50 min)                   120.00
Group Therapy                     (90 min)                           50.00
Parent/Therapist Consultation  (50min)                     120.00

Court preparation and/or appearance                      250.00 per hour

If  you are in financial need, or unable to meet the above stated fees, ACTSolutions will provide a “Sliding Scale.”  This must be discussed with your counselor.  The Initial Assessment fee is a set fee of $120.00.
